Observation: * Janitorial mop sink was removed with out approval; operator reminded that they need health approval to make alterations or remove equipment prior to doing so
Corrective Action(s): * Reinstall mop sink in original/approved location so that it is easily accessible for janitorial cleaning as using hand sinks or other food related sinks is not sanitary practice

Observation: * Some storage areas were found to be disorganized and food equipment/food stored in a manner that can lead to cross contamination; operator reminded to keep all storage areas in organized/sanitary manner to reduce cross contamination; also, light missing cover in dishwashing area and operator was verbally reminded to replace this last inspection
Corrective Action(s): * Ensure all storage areas maintained in sanitary/organized manner to reduce risk of cross contamination; replace missing light cover in dishwashing area and ensure all lights have proper covers to protect food/food equipment below if light fixture damaged

Observation: * Some walls in storage areas have deteriorated and require repair, etc., to food grade surfaces for easy cleaning/maintenance; discussed some areas that require repair/upgrade; operator to address all wall surfaces and other areas where deterioration evident
Corrective Action(s): * Ensure all walls/floors, etc., free of deterioration and food grade, easy to clean/maintain; repair and upgrade all of these areas

Observation: * Hand washing station was removed without approval; operator added a front wall without approval and no easy access to hand washing at front area; operator serving pizza slices without proper hand washing station accessible there; this is unacceptable as hand washing must be easily accessible; with Covid-19 pandemic hand washing must be even more accessible/available for more frequent hand washiing
Correction: * Ensure hand washing station on new floor plan easily accessible at front counter; remove pizza oven there as hand washing not available

* Operator re-installed second hand washing sink near pizza prep table area, etc. and as per approved floor plans; re-install soap and paper towel dispenser at hand sink
* Some storage areas were found to be disorganized/unsanitary so food and non-food stuff being stored together; operator to reorganize and maintain in sanitary manner
* Note: operator to re-install mop sink in original location as per original approval
* Note: light cover missing in dishwashing area; operator to ensure all lights have proper covers and maintain these
* Operator indicated they want to expand menu; discussed to submit updated menu items; also, discussed walk-in cooler storage concerns and that they would need to reduce current menu if wanting to add other menu items; also, discussed organization/maintenance/repair issues that need to be addressed prior to getting approval for new menu
