Observation: 1. **REPEAT** Dried food build up observed on blade of table mounted can opener
2. **REPEAT** Food products stored in walk-in cooler are not covered with lids.
3. **REPEAT** Tomato sauce observed to be stored in opened original can. Opened tin cans can rust where the lid has been cut open, contaminating the product.
4. Washed containers observed to be stacked while wet; water droplets trapped between containers
5. Raw chicken observed to be stored on floor of walk-in freezer
**Corrected during inspection** Raw chicken moved when identified.
6. Dough mixer observed to have dough and flour build up. Staff stated mixer is cleaned approximately twice a week.
Corrective Action(s): 1. Equipment needs to be cleaned and sanitized after use. Develop a cleaning checklist to ensure all areas are cleaned.

- Scissors, knives and can opener near hand washing station observed to be stored while dirty (Corrected during inspection)
- Dirty equipment observed to be stored as clean near 3 compartment sinks
- Tool cart observed to be lined with heavily soiled and damaged foil

Observation: 1. **REPEAT** Cardboard observed to be used as shelf liner for spice rack / dry storage. Cardboard appears to be soiled with grease/oil
2. **REPEAT** Sponge-like material (foam?) observed to be lining space bewteeen the two 3 compartment sinks. Parts of the sponge material have blackened.

If this does not exist in food safety plan, develop a method to properly clean and sanitize oversized or clean-in-place equipment and submit to district inspector.
