Follow-up inspection for inspection conducted on Sep 21, 2018 (RPT#: JMAA-B4TS5X) to assess for rodent activity and control methods. - 4 Ketch-All(TM) traps available in facility. Deceased mouse observed in 2 of the 4 traps, which will be removed by professional pest control company - No visible / recent rodent droppings observed at time of inspection - Pest control company arrives every 2 weeks to check for rodent activity and reset traps - Invoice for most recent inspection on Sep 25, 2018 shown to EHO at time of inspection. Indicated that all traps were checked, and removed 2 mice from kitchen area. Based on their inspection, approximately 2 mice are being trapped every week Facility is in good sanitary condition. Continue to monitor for any emerging entranceways, holes, or signs of pest activity (e.g. droppings, gnaw marks, live/dead rodents in traps, ripped dry storage bags). A spot check may be conducted between now and the next routine inspection.
