Observation (CORRECTED DURING INSPECTION): 1) -Quats sanitizer dispensing <50 ppm quats sanitizer.
2) -Inserts are stacked wet and put away wet after washing and sanitizing.
Corrective Action(s): 1) 200 ppm Quats sanitizer required
-Replace near empty sanitizer bottle and adjust dispenser tubing to remove air bubbles. *Test concentration each time*

-All other surfaces and hard to reach areas.
Corrective Action(s): Store all items at least 6" (15 cm) off the ground to facilitate cleaning and pest monitoring.
