Observation (CORRECTED DURING INSPECTION): Dead ants observed in the bottom of open bottles of rum and whiskey.
Corrective Action(s): All alcohol was discarded. Ensure all open bottles of alcohol is kept sealed to prevent the attraction of pests.

Observation (CORRECTED DURING INSPECTION): Knife stored clean on knife rack observed with dried food debris.
Corrective Action(s): Knife was removed and washed. Ensure all food contact surfaces are cleaned and sanitized after each use to prevent cross contamination.

Observation (CORRECTED DURING INSPECTION): 1. Bags of sprouts and celery observed stored directly on floor of walk-in cooler.
2. Rice scoop observed stored in an insert of water in front bar area.
3. Scoop with no handle observed in bin of sugar in preparation area.
4. Class bottle of water stored inside ice machine in kitchen area.
Corrective Action(s): To prevent physical and biological cross contamination of food:
1. Ensure all food and food contact surfaces are stored at least 6 inches off the floor.
2. Ensure rice scoop is stored dry and washed every 2 hours or stored in container of ice at 4 C.
3. Ensure all scoops with handles are used and stored outside bins in a sanitary manner.
4. Do not store any food, bottles, or utensils in ice machine.

Observation: Old dead cockroaches observed under bar shelving, beside hot water tank, and in staff bathroom. Pest control has been previously notified of issue. Pest control reports presented during inspection.
Corrective Action(s): Pest control company was notified during inspection. Spray all infected areas with chlorine water solution and remove insects wearing gloves. Send the most recent pest control report to the undersigned by next inspection.

- behind chests freezers located in kitchen and front bar area
- under ice machine
4. Back kitchen observed open during inspection.
Corrective Action(s): To prevent the attraction of pests clean all the areas listed above by removing any water, grease, and long standing debris. Keep back door closed at all times.
