Observation: Various food items (containers of raw meat, containers of cut vegetables, etc) in prep coolers across cooking line, and walk in cooler, were uncovered at time of inspection.
Corrective Action(s): Cover all food items when not in use. Inspection was conducted during a non-busy time; all items need to be covered and protected from contamination.

Observation: Operator does not have pest control report on site for most recent visit. Operator provided an invoice/receipt from their pest control company's visit, dated July 11, 2016. The invoice only indicates that pesticide was applied in an attempt to target cockroaches. There is no report indicating what observations the pest control company had, or if any of the mouse traps were serviced by them.
Corrective Action(s): Ensure to obtain pest control reports at time of visits and keep them on-site at all times.
