Observation (CORRECTED DURING INSPECTION): No liquid soap available at the hand sink by the walk-in cooler.
Corrective Action(s): Provide soap at this hand sink but also ensure that all hand wash stations have constant supply of liquid soap and paper towels.

Observation (CORRECTED DURING INSPECTION): Some food items in the walk-in cooler and dry batter in the kitchen area were left uncovered. Also, some food items were found directly on the floor. Some non-food items (staff personal belongings) were found on the shelving units where foods are stored. Improper utensils used for scooping sauces. Ice machine has slime on the surface.
Corrective Action(s): Discard all food items that were left uncovered. Keep all food items off the floor and all non-food items in the staff area. Use proper utensils for dispensing food items. Empty out the ice machine and clean and sanitize inside before making ice.

Observation: Droppings were found in the food storage area and also in the ware washing area.
Corrective Action(s): Clean and sanitize all areas contaminated with droppings and ensure pest control monitoring is in place.

Observation: Filth, grime and debris and were found in floor, shelving units, equipment (inside walk-in cooler/freezer, racks, prep tables, etc.)
Corrective Action(s): Clean all areas of the kitchen thoroughly. Regular cleaning required to maintain kitchen in sanitary condition.

Observation: Milk crates were used for storing food equipment and also sour cream/ice cream buckets are not to be re-used.

Walk-in freezer was at -18 deg C.
Walk-in cooler was at 3-4 deg C.
Prep line coolers were at or below 4 deg C.
Low temperature dishwasher had 100 ppm residual at the plate surface.
Temperature log was available.
Bar area:
Hand sink provided with liquid soap and paper towel.
Coolers were at 4 deg C.
Glass ware washer had 50 ppm.
Note:
Provide pest control invoice and report for pest control monitoring (daily, weekly, monthly).
Operator has voluntarily closed the kitchen for a few hours to clean.
Follow-up in the afternoon as well as tomorrow (January 16, 2019).
