304 - Premises not free of pests [s. 26(a)]
Observation (CORRECTED DURING INSPECTION): New rodent droppings observed in following locations:
1. In the corner of the janitorial room to the left side of the entranceway
2. In the downstairs bar in the lower cabinet areas (approximately 4-5 droppings in each cabinet compartment and behind the tray where spilled debris was observed (across from the glasswasher)
Corrective Action(s): The rodent droppings were removed with a 1:10 dilution of bleach and wiped with paper towels, then discarded. Continue to work with the professional pest control company to monitor for pests and determine potential entranceways that require sealing. Ensure that cultural controls such as sanitation are maintained - all spilled food debris must be wiped off before the end of shift.

Observation: Multiple deficiencies observed:
1. Area behind tray across from glasswasher contained food grime and spilled debris, as well as new rodent droppings.
2. Upstairs bar contains some remaining debris on the floor underneath the bar sinks. Grime inside the bar cooler was not wiped away.
Corrective Action(s): 1. The area was thoroughly cleaned with paper towels containing a 1:10 dilution of bleach, then immediately discarded.
2. Remove the debris from underneath the bar sinks, clean and sanitize the bottom of the bar cooler.

Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)] (Cooked chicken, meats, and eggs were stored at 6-7C in stand-up glass cooler and prep cooler underneath hot-holding units / microwave. They were moved to the coolers in the late morning (~2 hours ago) )

Follow-up inspection to routine inspection conducted March 19, 2018 (RPT #AWZUSV).
- Temperatures of stand-up glass cooler and right-side prep cooler both at 4C. Cooked meats and chicken at 4C at time of inspection. Temperature logs are maintained. Ensure that corrective actions are being done if the coolers cannot maintain 4C or lower during routine temperature checks. Take out only small batches of chicken and meat from the walk-in cooler to the prep coolers to improve circulation in the cooler
- Large ice machine is now in sanitary condition
- Sanitation in upstairs bar has been significantly improved - additional corrective measures need to be taken (see above)
- Iodine glasswasher in upstairs bar now contains 12.5 ppm iodine residual - good
- Professional pest control company on-site to install 17 mechanical traps and fly traps

Next routine inspection scheduled in 6 months; however, follow-up inspections may still occur to ensure that integrated pest management program is being maintained and that facility is in sanitary condition.
