Observation: High-temperature dishwasher is not functional due to leaking pipe.
Corrective Action(s): Repair the dishwasher and ensure that it can reach a temperature of 82C at the manifold (180F) or 71C (160F) at the plate level. Re-sanitize all dishes through the dishwasher before the facility undergoes a follow-up inspection. Closure order is currently in effect.

Observation: Rodent droppings observed around corner of facility underneath the "Pro-Line Advanced Nutrition" banner.
Corrective Action(s): 1. Remove the droppings with a 1 part bleach, 9 part water solution. Wear personal protective equipment and ensure that you saturate droppings for 15 minutes prior to removal.
2. Contact your professional pest control company (according to the operator's sister, the company has not arrived for 6 months) and discuss about an appropriate integrated pest management strategy. Currently there are numerous snap traps and bare adhesives lying around, some of which are quite old. You will need to discuss an organized system of monitoring and trapping rodents.
3. Remove all rodenticide stations from your facility. Rodenticide bait stations are not permitted inside.

Observation: 1. Significant amount of clutter observed at back storage area around chest freezer, as well as the area around the "Proline Adhesive Nutrition" banner.
2. Sink area and area around all panini grills need significant cleaning and sanitation improvements.
Corrective Action(s): 1. All clutter must be re-organized in the facility.
2. Remove all old snap traps, adhesives, and rodenticide bait stations. Contact your pest control company to implement a new integrated pest management strategy, including using appropriate mechanical traps.

CLOSURE ORDER issued for facility due to non-functional dishwasher and sanitation issues. Pipe inside has leaked and no hot water is able to be dispensed. No food preparation or dispensation is allowed for the public. Read the above violations carefully, as well as the CLOSURE ORDER on conditions for re-opening your food service establishment to the public.

Remain closed to the public and schedule a time for a follow-up inspection once the conditions noted on the CLOSURE ORDER have been satisfied.
