CLOSURE ORDER RESCINDED
- cleaning completed; ensure cleaning is maintained
- rodent droppings have been cleaned
- pest control has been in - forward a copy of the pest report to the undersigned
- follow all recommendations; plug all points of entry

- sanitizer measured 200ppm quats
