Follow-up inspection conducted to routine inspection and the following observations were made: - All utensils were maintained in sanitary condition. - Vent hood was cleaned and serviced. Next service due in Dec 2021. - Top of the oven was clean and had no visible food debris. - Dough mixer was found in sanitary condition. - Temperature logs were maintained and found up to date. - Staff had FoodSafe Level 1 Certificate. - Back door was still wide open. Screen door was in place but not closed at the time of inspection. **Keep the screen door in the back closed at all times to present entry of rodents/pests into the facility. - No Operating Permit was posted at the time of inspection. **The Operator requested for new decal instead of replacement Permit. A copy of Replacement of Permit application form and Refund form is emailed separately. Please submit the completed forms to the Health Authority by July 2, 2021. - Food menu and updated Food Safety Plan was sent to the Health Authority for three dishes but did not include all different types of food that will offered as per the Food menu. **Provide an updated Food Safety Plan by July 5, 2021.
