Observation: Permit is not posted in a conspicuous location. Operator kept the permit in the office drawers.
Corrective Action(s): Post permit on the wall where customers can see. Can be by soft drink area, on the wall under the menu or any other wall where the permit can be seen.

Observation: -Ice machine has build up of mold in the upper unit above the ice maker, in corners, and around the cover and in the ice bin on the shield and under the lid.
- Noted rack of biscuits beside rack of raw chicken without proper barrier- too close and cover was torn
Corrective Action(s): -Discard the ice from the ice machine, clean/scrub the ice machine, and sanitize with 100-200 ppm bleach sanitizer. Clean/sanitize ice machine regularly to prevent contamination of ice that is used for beverages- cleaning was in process at time of visit
- Move the rack ( biscuits ) to be away from rack of chicken and change the torn covers-done at time of visit

Observation: Poor cleaning/sanitizing of wall, floors, 3 compartment sink, shelving units inside cooler, inside and outside of cooler door, and on shelving units and on floors for food storage.
Corrective Action(s): Conduct deep cleaning of facility and emphasis on the areas observed. Ensure premises is regularly cleaned and sanitized to maintain sanitary conditions.

*NOTE: General sanitation needs work, clean and sanitize premises more regularly
*NOTE: Clean ice machine twice a month instead of once a month
*NOTE: Use ice and water to store rice scoop and mashed potato scoops
*NOTE: Use paper logs to keep track temperatures in walk-in cooler and freezer
*Recommended to have a shield by pop dispenser to stop splatter onto cups and store ice scoop in separate container

- Hand washing station equipped with hot/cold running water, liquid soap, and paper towels
-Good hand washing and glove change noted by staff
- All coolers at 4C or below
- All freezers at -18C or below
- All hot holding is above 60C
- Thermometers are available for temperature monitoring
- Temperature logs are maintained digitally
- 3 compartment sink used for dishwashing. 100 ppm bleach sanitizer measured in compartment for sanitizing
- Sanitizer buckets are used for wiping cloths (ensure sanitizer buckets are made at the beginning of each shift and changed regularly to maintain sanitizer concentration)
- Food storage is 6" off the floor, sticker labels used to indicate date food should be used by
** Floor tile in walk-in cooler by the drain is now loose (please repair)
** A couple rodent dropping noted around dry storage area- not a full inspection done due to conditions of the facility, have a full assessment done by the pest control to verify the current condition with regard to pest activities and rectify identified structural issues noted by pervious pest control report dated April 3-2024 or March 4 2024 ???
