304 - Premises not free of pests [s. 26(a)]
Observation: A) A variety of flies observed in the rea storage area and dishwash station. Screen on back door in poor construction or state of repair.
B) Mice dropping noted in many areas of kitchen (significant) including on some shelves/food racks and along walls in many parts of premises. Dead mouse observed on one of the sticky traps. An assortment of sticky traps placed, some which are soiled with food debris and others poorly positioned or moved.
Corrective Action(s): A) Eliminate the flies from entering the food premises. Contact pest control if unable to manage.
Fix the back door screen such that it is an effective barrier to fly entry OR keep the back door closed at all times.
B) Have pest control contractor re-visit premises and provide complete list of actions and recommendations needed to remove mouse infestation including recommendations that target blocking pest entry. Replace traps and bait stations. Adjust on-site frequency accordingly until infestation is eliminated.
**Re-inspection to take place to review above on date noted on report.

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: Many areas in the kitchen required improved cleaning and sanitation as a means to be cleaner and to reduce pest activity in premises. These areas include, but are not limited to:
- Walls observed with old splatters of food, especially in high food prep areas;
- Below grill and frier station and floor under counters in the same;
- Behind food and other containers along walls in all parts of kitchen;
- shelves and racks in storage areas, especially along rodent pathways and along walls;
- Walk-in cooler where organization needs improvement so food containers are not stored directly on floor.
Corrective Action(s): Clean all above noted areas and make sure the mouse dropping are all removed. Keep things cleaner.
This will be reviewed at follow up inspection date.

307 - Equipment/utensils/food contact surfaces are not of suitable design/material [s. 16; s. 19]
Observation: Ceiling tile replacement throughout the kitchen area, however, it was replaced with incorrect accustical tile, not food grade washable ceiling tile as required.
Corrective Action(s): All the ceiling tile in the kitchen area must be replaced with tiles that are washable and rated for food premises.
**This must be completed within 60 days. A follow up inspection will be completed.
