Observation (CORRECTED DURING INSPECTION): Trays of cooked chickens were cooling on a wrack. Internal temperature measured at 14-15C. Staffs were not able to demonstrate proper cooling procedure nor cannot recall when the batch was made. No time tracking in place. Chicken discarded at this time. Going forward, all cooling foods to be time tracked by placing a tape and write a time the food was cooked.

Observation (CORRECTED DURING INSPECTION): 1. Boxes of delivered raw meats observed in the kitchen corner. All potentially hazardous foods were relocated to a cooler at this time.
2. Containers of raw chicken thighs stored in an ambient temperature (internal temperature measured at 14C). Chicken thighs relocated to a cooler at this time.

Observation (CORRECTED DURING INSPECTION): A half tray full of cooked spring rolls in a hot holding unit was measured at 37.8C internally and held it for >2 hours. Operator discarded spring rolls at this time.

Observation (CORRECTED DURING INSPECTION): A rice scoop is stored in an unsanitary condition (by the entrance of the kitchen). The scoop now stored in an empty clean container.

Observation (CORRECTED DURING INSPECTION): Low temperature dishwasher's sanitizing cycle measured 0 ppm chlorine at the plate level. 3 compartment sink manual dishwashing set up prepared at this time.

Issue identified at this time. Sanitizing cycle measured at 50 ppm chlorine at the plate level.

2. Observed cardboard boxes that are reinforced with duct tapes being used as a food container. Operator discarded cardboard boxes at this time.

Observation: Observed fresh looking rodent droppings in the second floor storage area. Pest reports are not on site for review.

Observation: 1. Observed trays of baked breads in the second floor storage area that were intended for personal use (non-human consumption).
2. Observed number of boxes stored directly on the floor (boxes of oil and boxes in dry storage are).
Corrective Action(s): 1. Do not store any foods that are not appropriately covered on site to discourage pest activity. Correct this by: today.
2. All items on site to be stored >15cm off the floor to facilitate cleaning and to better monitor for pest activities. Correct this by: 2 weeks.

Observation: 1. Observed oil soaked cardboard box on the floor by the deep fryer. Operator removed the cardboard box at this time.
2. Observed number of flattened cardbaord boxes lining shleves in the second floor storage area.
3. Observed pool of standing water under the dishwasher.
Corrective Action(s): 2. Remove all cardboard boxes.
3. Clean and sanitize area under the dishwasher and any other standing water, if located.

Observation (CORRECTED DURING INSPECTION): Observed coolers and freezers (black plastic handles) were held together by duct tapes.
Corrective Action(s): Remove all duct tapes. All surfaces must be smooth, non-absorbent and easy to clean. Correct this by: 1 week.
