Observation (CORRECTED DURING INSPECTION): Internal temperatures of food items stored in 2 door under counter prep cooler at temperatures of 7-8 degrees Celsius.
CORRECTED DURING INSPECTION - items relocated to walk-in cooler unit.

Corrective Action(s): This cooler unit appears to be malfunctioning. Have unit serviced. Keep food items on ice in cooler unit during meal prep and return all items to walk-in cooler after food preparation. No potentially hazardous food items should be kept in this unit for longer than 2 hours and none without ice batch. The unit needs to be serviced IMMEDIATELY as this unit is critical to the operation of the facility.

301 - Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
Observation: 1) Rodent droppings found:
- On counter top under cutting boards
- On lower shelves of 2 of the 3 brown rolling racks
- On lower shelf of stainless steel grill storage unit
CORRECTED DURING INSPECTION - areas cleaned and sanitized with 200ppm Chlorine sanitizer, two cakes/loaves discarded during inspection as these were stored in this area, cutting boards cleaned and sanitized in dishwasher.

2) Sanitizer solution in all spray bottles and areas of kitchen found to be in concentrations of less than 200ppm. Sanitizer dispensing unit malfunctioning. Unit is dispensing sanitizer solution at concentrations of less than 200ppm.
CORRECTED DURING INSPECTION - 200ppm chlorine solution was created upon instruction of EHO. Surfaces re-sanitized with chlorine solution.

Corrective Action(s): 1) Ensure all surfaces are sanitized prior to the start of the work day. It is evident that rodents are able to climb to counter height therefore staff can not assume the surfaces are in a clean and sanitary condition because they were cleaned and sanitized the previous night. Morning sanitizing of counters should be occurring due to severe rodent infestation.

2) Due to the severe rodent infestation it is critical that sanitizer solution is checked daily. Staff mentioned that she had informed Chef of malfunctioning unit however an alternative sanitizer solution was not obtained until directed by EHO. Ensure staff are adequately trained to 1) check sanitizer solution daily with test strips, 2) if machine malfunctions to alert Management, 3) Management to put immediate request to service provider for repairs, 4) Management instruct staff to dump all old sanitizer solution and switch to alternative until such time as sanitizer dispensing unit is repaired.

Ensure Management and staff discuss the issues found today. The facility has a severe rodent infestation and serves an at risk population. The lack of adequate concentration of sanitizer solution is unacceptable.

Observation: Facility continues to have severe rodent infestation. 2 dead rodents found in one trap. Facility has active pest infestation.

Corrective Action(s): As infestation is not limited to the food preparation and storage areas but is throughout the building the facility must monitor all floors and take on a building Pest Management plan. Seek out Pest Control companies willing to inspect the building and seal holes or hire contractors to take on this task. The problem is not limited to the kitchen and will never be under control until the building has had proper inspections by Pest Control.

Observation: 1) Door sweep into dry storage area has been chewed at corner.
2) Dry storage room has items not stored in proper pest proof containers

Corrective Action(s): 1) Replace door sweep. As this is a reoccurring issue it may be time to invest in a more solid sweep (such as metal)
2) Obtain pest proof containers for those remaining dry food items (cereal, chips, etc) and dispense

308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
Observation: 1) Sanitizer dispensing unit is broken. Technician serviced during inspection.
2) Under counter 2 door prep cooler is malfunctioning. Unit could not achieve temperatures less than 7 degrees during entire inspection. Staff do not appear to be correctly checking cooler temperatures as every temperature for the past 17 days was identical.

Corrective Action(s): 1) Ensure sanitizer dispensing unit is repaired. Until such time as unit is repaired manually make sanitizer solution. Either 200ppm QUAT or Chlorine (bleach) based solutions.
2) Have 2 door under counter prep cooler serviced. Do not store potentially hazardous food items in unit for longer than during meal prep and have all items in ice baths. This is not a long term solution and needs to be corrected IMMEDIATELY. This unit is critical to the operation of the facility. Ensure no food item is stored in cooler unit without ice baths or longer than the duration of meal preparation. Ensure staff are actually checking temperatures daily.

EHO to return March 22 to ensure both cooler unit and sanitizer dispensing unit have been repaired.
