304 - Premises not free of pests [s. 26(a)]
Observation: Heavy rodent infestation discovered. Droppings found from the front of the restaurant to the back. Extremely heavy accumulation found around pizza box (wrapped) storage and dough mixer. A dead mouse found underneath prep station beside stove. Chew holes and insulation found along both East and West walls of restaurant. Some droppings found on upper shelves above pizza coolers where you store bags, foil, and condiments.
Corrective Action(s): The following must be addressed:
- Deadline 2 weeks - call back your best control company to fill all open holes and install door sweeps along all interior and exterior doors.
- Deadline 1 week - A DEEP clean of the restaurant must occur. This means moving away all heavy equipment from the walls, scrubbing away old oil & grease, and cleaning off food debris & splatter from every surface. You WILL require help for this task given the 1 week deadline.
- Immediately - Put away all food items. Make a bucket with 1 part bleach and 9 parts water. Use this solution to soak all of the droppings, then wipe them up while wet.

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: General sanitation of facility poor, with food splatter & debris observed on numerous surfaces, food and oil under heavy equipment, clutter behind chest freezers, and generally poor structural maintenance. The above listed problems have led to a heavy rodent infestation and must be fixed.
Corrective Action(s): As noted in the pest violation, a thorough cleaning is required of the entire facility. The deadline for this cleaning is within 1 week.

Please see above violations. If they are not corrected within a timely manner, Correction and or Closure Orders will be issued.
No contaminated or chewed products found on site. Take special care to use pest proof product bins for food, and store as much as possible in your walk in coolers or freezers.

A pest control report was on site from yesterday. 45 traps were allegedly set, with at least 1 found and removed with a dead rodent. As noted above, have your pest control come back to fill additional holes AFTER you've properly cleaned. Your pest control technician must be able to access wall space behind equipment, and along the wall near the mixer.
