Observation: 1. Lids for take out containers and food equipment located too close/underneath the soap dispesner and paper towel dispenser. This could result in risk of contamination due to splashing. This is a repeat violation.
2. Condiments, salt/pepper pouches and take out containers found to be stored in a cabinet right underneath the hand sink.
This could lead to contamination from potential leakage of the drain.
Corrective Action(s): 1. You must relocate either the equipment and take out utensils or the soap/paper towel dispenser immediately (By July 14, 2020).
2.Relocate all take out containers/lids and food items to an alternate location ( Immediately).

COVID-19 protocol/Order follow up.
The seating in the front section of the store must be re-arranged to ensure there is at least 6 feet distance between patrons.
You must remove or relocate the middle table ( immediately).

Walk-in cooler below 4 C.
Walk-in freezer below -18 C.
Main preparation cooler below 4 C.
Internal temperature of sandwich ingredients < 4 C
Beverage cooler < 4 C.
Hot holding of meatballs and sauces> 60 C.
Liquid soap and paper towels available at the hand sink.
Quats sanitizer at the dispenser and in spray bottle tested 200 ppm.
Temperature logs are up to date.
