Observation (CORRECTED DURING INSPECTION): Sanitizer pail had 100 ppm QUATs at the time of inspection.
Corrective Action(s): Operator restocked sanitizer pails with 200 ppm QUATs at the time of inspection. Recommend exchanging solution every 2-3 hours and keeping the wiping clothes submerged in the sanitizing solution.

Observation (CORRECTED DURING INSPECTION): 1 dead mice found in a trap near the mop sink in the back area away from food prep area. 5 other active traps found on premise with no activity.
Corrective Action(s): Operator cleaned and sanitized area near the trap during the time of inspection. Pest control services provided monthly. Ensure to monitor traps daily.

Observation: Back screen door has a small opening on the right lower corner of the screen door.
Corrective Action(s): Ensure opening is covered up to remove any potential pest entrance. (Recommend installing door sweeper). Actively monitor the back screen door being close fully as it does not shut on its own.
