Corrective Action(s): Ensure the slicer is taken apart and all cracks and crevices are cleaned. Clean food debris that has fallen into the box.

Corrective Action(s): Remove the droppings and clean. Step up your pest control measures. Remove droppings from the tops of cans and clean with soapy water. Sanitize with bleach solution.

Observation: Gap noted under the back door.

Observation: Salad fridge is not working. Shelves in the walk-in fridge is rusty. The edge of the kitchen counter top is broken in several places and is in poor condition.
Corrective Action(s): Replace salad fridge. In the meantime, continue to place salad greens and dressings on ice. Discard greens after 2 hrs. Remove rust and paint the shelves in the walk-in. Replace the counter top.

After food is reheated, check with a thermometer to make sure that it is at least 165F/74C. If not, continue to reheat. Calibrate your thermometer.
Do not use black plastic bags to cover food. Use food grade plastic films when it is in direct contact with food.
Sack of onion is stored too close to the mop. Move the mop or store onions elsewhere.
Ensure chipper is thoroughly cleaned after use. Do not line chipping surface with plastic film.
Temperature of refrigeration, hot holding units were satisfactory.
Sanitizer detected in the rag soaking bucket and the dishwasher.
