Observation: Noted wiping rags on counter dirty- no sanitizer solution
Corrective Action(s): - Wiping rags to be washed and store in sanitizer solution when not in use.
-Rinse after use and store in a sanitizer bucket ( 1 oz of bleach in 1 gallon of water =100 ppm)

Observation: No sanitizer detected from dishwasher ( run twice)
Corrective Action(s): -Repair so enough sanitizer is detected at rinse cycle.
-All washed dishware to be sanitized by soaking in bleach solution ( 1 oz in 1 gallon= 100 ppm) for 2 minute and air dry

Observation: Some stuff appear to be to rodent dropping noted mixed with dirt in some areas.
Corrective Action(s): -Clean all and put sticky traps for monitoring for any possible rodent activities

Observation: - Lot of broken walls, fallen baseboard, broken tiles in entire facility.
- A lot of stuff on the floor in storage room and kitchen
Corrective Action(s): - Repair all possible point of entry for pests
- All stuff to be off of the floor on shelving to reduce the breeding and harboring for pests
