Observation: The light bulb in the walk-in-freezer could not be turned on by the switch. Staff mentioned that it was in good working order yesterday.
Corrective Action(s): Ensure the light bulb in the walk-in-freezer is in good working order; Correct within 1 month.

Comments

Permit to operate was posted however the old decal (Expiration date: March 31, 2016) needs to be replaced with a decal for this fiscal year (Expiration date: March 31, 2017).
Liquid soap, hot and cold running water, and paper towels were available at the handsink and 3-compartment sink.
Staff were adequately handwashing between handling money and putting on a new pair of gloves prior to preparing sandwiches.

Walk-in-cooler, sandwich cooler, under-the-counter cooler, and drinks cooler were 40 degrees F (4 degrees C) or less.
Walk-in-freezer was 0 degrees F (-18 degrees C) or less.
Hot-held soups and meatballs were 140 degrees F (60 degrees C) or hotter.
Refrigeration units were equipped with thermometers.
Temperature records were up-to-date.

200 ppm QUATS was available in the spray bottle and the third compartment of the 3-compartment sink.
Dishwashing flow was adequate in the 3-compartment sink. The soapy water and sanitizer were in a clean condition even after the lunch rush.
Food storage areas were clean and organized.
No signs of recent pest activity were observed.
Water was clearly labelled as "water" in the spray bottle.
