Observation: Mouse activity noted through out facility:
Kitchen:
1. Dry storage area - mouse droppings under and on shelves, bedding noted in staff shoe
2. Along wall and under counters near the dishpit and sauce cooler

Bar:
1. Under bar - droppings noted on shelves and floor
2. In bar cabinets - Sugar packages partly eaten/damaged, droppings

***Reviewed monthly pest control reports (3) from Terminex - most recent one available is Nov 2021.
*None of the reports have any detail about findings/observations, details of service, recommendations. Only basic service note and invoice information
Corrective Action(s): Concerns noted above of mouse infestation and concerns with the details shared about service provided by pest control company.
1. Clean all areas identified by pest activity ( droppings, bedding, damaged)
2. Discard damaged products, clean all surfaces and disinfect
3. Ensure all food products are stored in pest proof containers
4. Request more information on pest control reports from technician / company - ensure the service paid for is being done, done well and if they have any recommendations on what to improve.
5. Pest control service once a month is not sufficient while pest infestation is underway, more frequent service may be required until pests are controlled.

2. Lower shelves through out kitchen and bar

4. Under equipment throughout facility (Hard to reach areas)

Corrective Action(s): Ensure the above areas are maintained in sanitary condition. Proper cleaning will help identify pest activity and increase sanitation.

Observation: 1. No paper towel dispenser available to hold paper towel roll near kitchen hand sink.
2. Paper towel dispenser in staff washroom not functioning / dispensing paper towel
Corrective Action(s): Ensure paper towel is easily accessible to food handlers at handwashing stations in dispenser or paper towel holders. Loose rolls or piles of paper towels is not acceptable for long term practice for proper handwashing.

1. 1 missing a label completely (200 ppm QUATS detected)
2. 1 bottle has wrong label (Orange solution labelled as disinfectant, same label as purple disinfectant solution bottle).
Corrective Action(s): Ensure all chemical containers are properly labeled with its contents.

2. Change Orange solutions bottle label to its proper label. Kitchen staff unsure what the orange solution is.

***Fire Hazard Caution: ensure dry in dry storage room is properly maintained to avoid becoming a fire hazard due to its high heat and age.

-Tables approximately 2m apart - ensure back of seats are at least 2m apart or physical barrier is in place
