Observation: 1. Green rodent bait (powdered and bar) observed throughout facility outside of bait stations (e.g. under prep station, near food storage)
2. food stated to be not for human consumption observed on cooling rack. Droppings were on this product.
Correction: 1. Discard all food, remove bait and clean area thoroughly. Rodent issue observed and may contaminated food.

Received call from operator of store stating facility has been cleaned.


CLOSURE ORDER RESCINDED.

The facility has undergone thorough cleaning and disinfection. Significant improvement from last time.
- rodent droppings from the retail sales area had been removed and the surfaces had been disinfected.
- back area also undergone deep cleaning; loose rodent bait removed and cleaned up

- items not required for operation removed from facility

- professional pest control company contracted to monitor pest issue 2-3x/week
- please provide a report to inspector weekly
- follow recommendations and instructions from Orkin to resolve pest issue.

Monitor facility for pest activity:
record down areas where activity is sighted and inform pest control company, and
clean/sanitize areas as necessary

All food contact surfaces and equipment should be cleaned and sanitized prior to using daily until pest issue is resolved to ensure sanitation.
