HAZARD RATING FOR YOUR FACILITY: High (≥30) Total Score = 33
Critical Hazards: Total Number: 1
111 - Semi critical items not adequately cleaned and disinfected
Observation: (code 211) Metal foot file (paddle/rasp) for callus removal is cleaned and then disinfected with alcohol which dries quickly and is not a high level disinfectant.
Corrective Action(s): Immediately use a high level disinfectant as per directions on the chemical bottle label to properly disinfect foot file/paddle which is a semi critical item. Provide a container with lid that is large enough to store foot paddle for the required contact time in chemical disinfectant. Provide disinfectant with expiry date on label. Watch dates.
Violation Score: 15

Non-Critical Hazards: Total Number: 2
219 - Non critical items not adequately cleaned and disinfected
Observation (CORRECTED DURING INSPECTION): Cleaning procedures for foot baths after each client include disposal of single use plastic liner (good), then spraying with chlorine disinfectant while the water is running. The chlorine concentrtion is at 1000+ppm but it is not being used effectively to disinfect as it is mixing with water and going down the drain.
Corrective Action(s): Ensure all staff follow procedures: 1. Dispose of plastic liner. 2. Clean the plastic tub with detergent. 3. Rinse with water. 4. Disinfect with a disinfecting solution. If using bleach, use 1 tsp domestic bleach for every 1 cup of water and make fresh daily. Let sit for 10 minutes. 5. Drain. Label all chemical bottles to accurately show contents.
Violation Score: 9

822 - Adequate hand washing stations and/or hot and cold water not provided
Observation (CORRECTED DURING INSPECTION): There is no hot water in treatment room #1 used for eyelash extension services.
Corrective Action(s): Hot water was turned on during inspection. Keep hot water on at all times when room is being used so hands can be washed properly.
Violation Score: 9
