Observation: Back door and front door have gaps between the doors, when closed and gaps beneath the doors which allow for entrance of pests.
Corrective Action(s): Doors are to be pest proofed with weather stripping to close off gaps to pest proof premise.

Observation: Mouse droppings throughout dry storage room on floor/corners. Dead mouse on glue board in dry storage room. Food debris collecting under shelving in walk in cooler.
Corrective Action(s): All areas to be thoroughly cleaned to remove all debris from surfaces.
Areas affected by pests are to be sanitized with dilute bleach after cleaning.
Bottom shelving in walk in cooler to be adjusted 6" off floor to allow for easy cleaning.
