Observation (CORRECTED DURING INSPECTION): Pizza toppings such as ham, salami and sliced mushrooms are overfilled and measured at 8C in the cooler with loose cover and wooden cover.
Corrective Action(s): Please reduce the inserts to half only. Repaire/replace cooler cover so there is no gap when the cover is in use. Wooden board is insufficient.

Observation: Blender appears to have mildew and build up inside, photos taken.
Corrective Action(s): Please thoroughly clean and sanitize the blender. This should be done more frequently.

302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation (CORRECTED DURING INSPECTION): 1) Low temperature dishwasher was dispensing 0-10ppm chlorine in the beginning of inspection. 2)Bleach sanitizer bottles were all over 200ppm. Several wet wiping clothes left on counter. Repeat.
Corrective Action(s): 1) Operator added bleach sanitizer and changed buckets for detergent and rinse aid, then ran the unit over 5 times until the dishwasher was dispensing 50ppm. Contacted technician for maintenance. Ensure there is 50-100ppm chlorine at plate level to achieve proper sanitizing effect. 2) Use 1/2 teaspoon bleach to 1 litre water for 100ppm. Remix all solutions and keep wet wiping clothes submerged in 100ppm between uses.

Observation: 1) Spinach, mushrooms and vegetables were covered with damp cloth. Repeat. 2) An insert of raw chicken stored on top of cooked pasta.
Corrective Action(s): 1) Do not use damp cloth to cover food. Use clean damp paper towels as discussed before. Discard paper towels after. 2) Relocate raw meats so they are stored on the bottom of ready to eat foods.

Observation: Two bags of frozen squid and one frozen chicken left on the table at 6C to be thawed.
Corrective Action(s): Do not thaw at room temperature. Place food in walk-in cooler overnight or under cold running water for immediate use. Operator placed frozen food back in walk-in cooler.

Observation: Rat droppings noted on floor behind chest freezer all along the back hallway. Ches marks of wiring noted in same area. Several fruit flies noted in front bar area.
Corrective Action(s): Increase sanitation, remove droppings and have pest control company identify access points. Seal all entry points and continue to monitor for pest activitiy through droppings removal. Operator removed rat droppings at end of inspection. Remove any moisture/organics around bar area. Keep damp cloth submerged in bleach sanitizer. Send pest control reports from September to date for review.

Observation: Food debris noted behind chest freezer, dough mixer, and racks of undercounter cooler across cooking station.
Corrective Action(s): Clean and sanitize all noted areas.
