NOTE:
- Continue to clean diligently each night
- Continue to trap for pests
- Continue to monitor for pests in front cupboards and around dishwasher
- Seal holes around dishwasher.
- Ensure Pest technician replaces and services traps around dishwasher
- Front sink is a handsink according to original plans submitted to FHA by DQ.

NOTE: Facility is to be chemically treated for pests. Ensure all single use utensils, paper towels, cleaning cloths, re-usable utensils, food items, Cake decorations, equipment, or any other items contacting employees hands or food contact surfaces are removed from area to be chemically treated. Store all items inside walk in cooler or storage locker. All permanent fixtures that have a food contact surface are to be protected from contamination (bagged, etc.). All food contact surfaces are to be sanitized prior to use. Consult with Pest Technician for additional preventative measures.
