Observation (CORRECTED DURING INSPECTION): Lower shelves of prep area in kitchen observed to have accumulation of rodent droppings. This would have been from previous night. Equipment intended for food preparation stored in this area.
CORRECTED DURING INSPECTION - Items removed and put through sanitizing dishwashing cycle.

Corrective Action(s): Active severe rodent infestation at facility.
DO NOT store any food or food equipment on lower shelves until such time as rodent activity is eliminated. Remove all dishes and equipment from lower shelves until rodent infestation is eliminated.

Continue to clean and sanitize these affected areas - all lower shelves in preparation area need to have items removed from storage, area cleaned and sanitized and all equipment run through sanitizing cycle on dishwasher.

Spray rodent droppings with 200ppm chlorine solution or equivalent. Remove droppings from area. Wet affected area with 200ppm chlorine or equivalent and soak area.

Observation: Facility continues to have active rodent infestation. Based upon observations today infestation is severe and requires immediate attention. Areas from previous night have rodent droppings again. One live mouse observed at facility

Corrective Action(s): Have facility inspected by Pest Technician immediately. Ensure thorough inspection occurs. Locate all entry points and follow recommendations advised by Pest Control Company Technician.
Nothing to be stored on bottom shelves.

Observation: Back door has gap along middle. This exit leads directly to exterior garbage area.
Holes observed inside storage areas of main prep line.

Corrective Action(s): According to owner back door requires full replacement. Discuss with contractor and determine if temporary solution can be made to seal gap until such time as door is replaced (door has significant delays due to pandemic)
Facility needs to be inspected by pest technician to determine entry points and sources of pests. Pull all items from walls to inspect for holes. Seal with non-chewable materials (metal, wood, etc.) as required.
Do not store items on lower shelves until such time as pest infestation is eliminated.
