Report of Findings
Community Care Facilities Licensing [CCFL] received a complaint on May 13, 2022, alleging concerns regarding Care and Supervision.

The allegations included the following details:
- A child had access to hazaradous materials

All areas of the complaint were fully investigated. Evidence was received through inspection, interviews and a review of relevant records.

Observation: Outdoor Playground: A large part of the peeled off wall and small pieces of the paint materials were accessible to the children.
