RECORDS AND REPORTING: 19070 - CCLR s.57(2)(a) - A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation: Licensing reviewed 3 children's files and it was found that 1 file was missing a record of the child's medical insurance plan number. Manager called the parent while Licensing was present and will obtain the information as soon as possible.
Corrective Action(s): Please ensure that all records contain complete and current information prior to attendance.
