Observation: * Some raw fish product was found being stored above ready to eat food product and this can lead to cross contaminaton; Also, some fly swatters found stored with dry product in cupboard and this can lead to contamination-- fly swatters are not approved for food service areas; aprons were found stored with personal stuff and some personal stuff was being stored on counters(eg. cell phone, etc)-- operator reminded to keep personal stuff stored properly away from food/food equipment (including tooth brush which was found at one of the hand sinks in the kitchen
Corrective Action(s): * Ensure all raw fish and meat stored below ready to eat or precooked foods; also, fly swatters are not acceptable in food service facilities in kitchen and storage areas; keep personal stuff stored properly away from food/food equipment including toothbrush for brushing teeth

Observation: * Build up of grime/debris found through out kitchen and storage areas including storage shelving, food equipment, etc.; Also, the cloth curtains were dirty and require regular washing, etc
Corrective Action(s): * Ensure all areas properly cleaned, sanitized and maintained in sanitary condition; Cloth curtains to be cleaned regularly as these were very dirty

Observation: * Coffee table was found to have metal wrap around the bottom shelf and this is not acceptable or sanitary; operator to repair table or replace with proper food grade table; also, newspaper was found being used on some tables in frying area and also in containers to store food equipment and this is not food grade material and can not be used on food contact surfaces or shelving
Corrective Action(s): * Ensure all tables, shelving in proper food grade working order and maintained in sanitary condtiion;

* Cold-holding units were 4'C/40'F or colder

* Food was covered in storage areas
* Monitoring records were available although operator had documented for 2 days in advance; operator reminded to keep accurate records
* Note: cleaning to be done through out kitchen and storage areas(including food equipment)
* Hand washing stations had liquid soap and paper towels
* Sanitizer was available in buckets storing wiping rags(200 ppm bleach solution)
* Note: ensure all raw meat/fish stored below ready to eat foods
* Note: newspaper not food grade so can not be used for storing food equipment and on shelving
* Follow up inspection to be done next week
