Observation (CORRECTED DURING INSPECTION): Delivery of frozen meats observed to be outside of cooler/freezer unit for 2 hours during the inspection. CORRECTED DURING INSPECTION.

Observation: 1) Take out containers found to be stored on shelf where rodent droppings were found. The take out containers were food contact side down.
2) Cutting board found to be stored on floor.
3) Scoops for beverage preparation area stored in dirty water in container.

Corrective Action(s): 1) Discard all contaminated take out containers. Remove cardboard liner from this shelf.
2) Do not store cutting boards on floor it is unsanitary.
3) Ensure all scoops for beverages and rice paddles are stored in containers with ice water and changed frequently to prevent against contamination.

Observation: 1) Storage containers used that are not food grade. Food has the potential to be contaminated by these food items not food grade. Chemicals from non-food grade items have the potential to leach into food. CORRECTED DURING INSPECTION - food items discarded during time of inspection.
2) Staff lunch observed in cooler unit
3) Personal items including cell phones, vitamins, clothing, shoes observed to be stored surrounding and on top of food items.
4) Scoops found stored in dry storage bins.
5) Cardboard used as shelf liner. Cardboard can not be cleaned and sanitized prior to use.
6) Block of ice/broth found thrown into freezer without container.
7) Raw eggs, raw meat found in cooler units to be stored above ready to eat food items.

Corrective Action(s): 1) DO NOT use blue buckets labelled as "LOWES". These buckets are not food grade and are for construction purposes only. Use only food grade containers suited and designed for food preparation. Blue buckets marked "LOWES" can be used for cleaning purposes only.
2) Staff lunches should have separate designated area away from main customer food
3) Personal items need an area designated for storage. No medicines/pills or cell phones on food preparation surfaces or stored in food production area.
4) Scoops are to be removed from dry storage bin every time they are used and run through dishwasher after each use
5) Cardboard is not to be used as a shelf liner
6) All food items in freezers are to be protected from contamination by being fully contained in a container
7) Rearrange all cooler units such that raw meat is below ready to eat food items.
8) Once opened all canned food items should be transferred to prevent against leaching of chemicals into food.

Observation: Three dead mice observed in trap.
Rodent droppings observed throughout facility.
Evidence of rodents in hot water storage area including chewing of cardboard.

Corrective Action(s): Remove all dishes and equipment from lower shelves until rodent infestation is eliminated.
Spray rodent droppings with 200ppm chlorine solution or equivalent. Remove droppings from area. Wet affected area with 200ppm chlorine or equivalent and soak area.
Inspect facility for points of entry for pests. Seal any holes. Use non-chewable materials when sealing holes. Inspect ceiling.
Inspect bagged food items for chewing or contamination. Discard any contaminated food items.
Inspect doors for gaps. Seal any holes in bottom of doors with door sweeps.
Discard contaminated single use items - the take out containers - that have been in contact or stored with rodent droppings.
Facility has been Ordered CLOSED. Cease all food production immediately.

- Floor behind dishwasher

Corrective Action(s): Remove all dishes and equipment from lower shelves until rodent infestation is eliminated.

Discard contaminated take out containers.
Ensure all dishes and equipment stored on lower shelves are washed and sanitized - either manually or through dishwasher.

4) Duct tape found to be used to hold chest freezer unit together.

Corrective Action(s): 1) Clean and repair this area. Use Paint that is durable and intended for metal.
2) Find cooler handle and repair with proper handle
3) Replace all cooler seals that are broken.
4) Remove duct tape and repair freezer unit.

CLOSURE ORDER ISSUED:

Reason for the Closure Order:

Violation of Food Premises Regulation Section 17 (1) Every operator of food premises must ensure that the premises and the equipment and utensils used on the premises are maintained in a sanitary condition. AND Section 26Every operator of food premises must ensure that the premises are (a) free of pests, (b) free of conditions that lead to the harbouring or breeding of pests, and (c) protected against the entrance of pests.

The following action must be taken:
1) Clean and sanitize all areas affected by rodent droppings. Remove all rodent droppings/feces from facility. All floors, shelves, equipment, utensils, dishes contaminated by rodent droppings must be cleaned and sanitized to remove rodent droppings. Remove all items from shelves and clean and sanitize. Move all items off floor to sanitize floor properly. Discard all contaminated single use items (take out containers). All surfaces (including floors, shelving units, and counters) must be sanitized with 200ppm Chlorine or run through dishwasher.
2) Inspect bagged food items for chewing or contamination. Discard any contaminated food items.
3) Fill any holes with non-chewable (i.e. metal or wood) materials. Seal exterior doors. Inspect remainder of facility for holes including ceiling.
4) Cease food production IMMEDIATELY. No food production is to occur during cleaning of rodent droppings to prevent against contamination
5) Leave Order posted. Do not move, alter, cover, destroy Closure Order once posted. Contact Fraser Health Authority for opening inspection of facility
6) Do not open facility without approval from Fraser Health Authority.
