Observation (CORRECTED DURING INSPECTION): The doors of the sushi bar cooler had been removed again. This is a repeat infraction.
Corrective Action(s): As discussed, the doors of the sushi bar cooler must NEVER be removed. The temperature of the fish and other foods in this cooler must be maintained at 4 degC or below.

Observation: Dishwasher rinse temperature after 4 tries is at 68 degC.
Corrective Action(s): Get the dishwasher fixed. It must be able to maintain a temperature of at least 71 degC at plate level. Operator called the repairman during the inspection.

Observation (CORRECTED DURING INSPECTION): Paper towel dispenser at the hand sink was empty.

Observation: Raw meats are being stored above vegetables in the cooler at the kitchen entrance.

-200ppm bleach solution is available in spray bottles for sanitizing counters.
-A pest control company is on a monthly contract.
-Miso soup was at 73 degC
-The flooring has been replaced. There's a backsplash behind the hand sink in the front.
