Observation: Ice machine in back preparation area observed with pink and black mold build up. Manager turned off the ice machine during time of inspection.
Corrective Action(s): Discard all ice in the ice machine. Clean and sanitize the ice machine with 100 ppm bleach solution (or as per manufacturer instructions) to remove mold build up. In the meantime, purchase bagged ice from an approved supplier or use an alternative ice machine on site. Ensure the ice machine is cleaned and sanitized more frequently as part of the regular cleaning schedule.

Observation: Few small flies observed in back mop sink area and donut preparation area. Pest control reports were available on site for verification of inspection.

- QUATS sanitizer in sanitizer pails with wiping cloths (X5) measured at 200 ppm QUATS concentration with test strip
- QUATS test strips available on site
- High temperature dishwasher measured at 82'C at plate surface
- 3-compartment available on site
- All food stored at least 6 inches off the floor
- All food observed covered
- Equipment stored in a sanitary
- Pest control company contracted to complete monthly inspections
