Observation (CORRECTED DURING INSPECTION): 3-door cooler was measured at an ambient temperature of 8 degrees C. Internal temperature of probed foods measured 8 degrees C. Note: Temperature records indicate cooler was measured at less than 4 degrees C this morning.
Corrective Action(s): Higher risk foods transferred to cooler at 4 degrees C or less. Ensure potentially hazardous foods are maintained at 4 degrees C or less to limit growth of potential pathogens and/or toxins over time.
Note: Cooler temperature started to drop slowly during inspection--continue to monitor and keep cooler doors closed in order to maintain cold temperatures. Have cooler adjusted or serviced if temperature does not return to 4 degrees C or less.

Observation (CORRECTED DURING INSPECTION): One spray bottle labeled as sanitizer contained between 0 and 100 ppm quaternary ammonium compounds (quats) sanitizer.
Corrective Action(s): Sanitizer bottle was emptied, refilled, and tested at 200 ppm quats. Ensure that sanitizer is maintained at concentrations required for effective sanitizing of food contact surfaces at all times (e.g. 200 ppm quats).

- Hand washing station is unobstructed and supplied with hot and cold running water, liquid hand soap, and single use paper towels.
- Staff members displayed proper hand washing and glove usage
- Equipment and utensils are stored away in clean condition
- Three compartment sink supplied with three sink plugs and hot/cold running water
- Quaternary ammonium compounds (quats) sanitizer is available at 200 ppm (dispenser, bucket with wiping cloths)
- Temperature and sanitation records are up to date
- No signs of pest activity observed at time of inspection
- Permit posted at front

Note: It was stated that sandwich table was turned off recently for defrosting and cleaning. Internal food temperatures ranged from approximately 4 to 8 degrees C. Inserts were removed and unit was cleaned at time of inspection, then turned back on. Ensure internal food temperatures are maintained at 4 degrees C or less. Temperature records show unit was at less than 4 degrees C today.
