Observation (CORRECTED DURING INSPECTION): * Sanitizer was not readily available or set up to use upon beginning of inspection. Operator told to set up sanitizer and have it ready to go prior to handling any food. Change sanitizer every couple hours or every food service time
Corrective Action(s): * Ensure sanitizer available at ALL times during operation (eg. in buckets to store wiping rags) and change every couple hours or every food service time

Observation: * Grime and build up of debris, etc., noted through out kitchen on food equipment and shelving etc., that requires cleaning attention for sanitary maintenance. Also, thoroughly clean ALL area under/behind food equipment and under walk-in cooler food shelving to remove build up of grime/debris
Corrective Action(s): * Ensure ALL areas of food equipment/flooring, etc., properly cleaned/sanitized thoroughly and maintained in sanitary condition. Reoranize ALL shelving areas through kitchen and storage

Observation: * Flooring under cooking line has deteriorated further since last inspection and requires immediate repair. This was cited on last inspection too. Operator said they would repair by fall 2017 but still not repaired. Flooring under dishwashing area also requires repair too and now water leak occuring under dishwashing area so that also requires repair for proper sanitation to occur
Corrective Action(s): * Repair flooring under cooking line immediately as this was cited last inspection and must be addressed immediately. Also, repair water leak under dishwashing area and flooring after that. This is required for proper sanitary maintenance to occur

* Cold-holding units were 4'C/40'F or colder
* Hot-holding units were 60'C/140'F or hotter
* Freezer units were -18'C or colder
* Dishwasher final rinse was found to be at 50 ppm chlorine residual at plate surface internally
* Sanitizer was set up once operator told to do so and was found to be 200 ppm quaternary ammonium concentration in buckets to store wiping rags
* Monitoring records were available and up to date
* Food was dated in storage; ensure ALL food covered in storage too
* Staff were dressed appropriately and had hair protection
* Note: ensure hand washing sink free of clutter/food equipment and accessible for use at ALL times
* Note: flooring repairs to be done immediately
* Follow up inspection: Mid-December 2017
