Ceiling lights were off when I entered the facility while food preparation was being done. Corrective Action(s): Ceiling lights must be on at all times when in operation to provide the correct illumination at food preparation surfaces. Repair hand sink. This is an outstanding item now recorded on 2 previous inspection reports. Violation Score: 15
