Observation: Rodent droppings observed in several areas of the store including but not limited to inside and outside the walk-in cooler and in the dry storage area. There are areas where the droppings and flour are caked onto surfaces as pointed out. There were several flies noted on the prep table in the back.
Corrective Action(s): Thoroughly scrub these areas clean and stay on top of cleaning. Increase the frequency of your pest control servicing to twice a month. Replace fly traps with new ones. Send me the service reports.

Observation: Gap noted under back screen door. The mesh on the door is not intact either.
Corrective Action(s): Provide a sweeper under this door and repair the mesh on the door.

Comments

On Follow-Up:
Operator has FS Level 1 equivalent training.
Some cleaning has been done but it's not enough.
Cooler temperatures are being recorded.
A new spray bottle with sanitizer is available for use. GET ANOTHER SPRAY BOTTLE FOR THE FRONT OF HOUSE. LABEL THE SANITIZER BOTTLES.
Floor under 3-comp. sink and mop sink has been sealed.

ANOTHER FOLLOW-UP WILL BE CONDUCTED NEXT WEDNESDAY. IF THE SAME AMOUNT OF RODENT DROPPINGS ARE NOTED NEXT WEEK, A CLOSURE ORDER WILL BE ISSUED UNTIL YOU CAN MANAGE YOUR RODENT ISSUES.
