Observation (CORRECTED DURING INSPECTION):
- No paper towel and no paper towel dispenser observed at the bar hand sink.
Corrective Action(s):
- Install a paper towel dispenser next to the sink. Ensure the hand sink is equipped with single use paper towels and liquid soap at all times.

Observation (CORRECTED DURING INSPECTION): Block of frozen sliced pork in plastic bag was being thawed in prep sink, with no water filled up.
Corrective Action(s): Ensure proper thawing practices are followed - thaw in cold water, in cooler or use microwave. Sink was filled up with cold water at the time of inspection.

Observation:
- Fruit flies observed in the dishwashing area and at the bar handsink area.
Corrective Action(s):
- Ensure there are no food residues and continuous water source in the area noted above. Install traps if necessary.

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation:
- Rodent droppings observed in the corner behind the tempura mix bin.
- Ketchup dispenser in the back kitchen observed with dried up ketchup residue on the nozzle.
- Food debris observed along the wall junctions. Oil accumulation underneath deep fryer.
- Mouldy caulking observed around the bar hand sink.
- Mould observed in the ice machine.
Corrective Action(s):
- Clean areas noted above. Ensure to clean all hard-to-reach areas after closing.
- The ketchup dispenser nozzle opening is big and is designed in a manner that residue can build up very easily. Change the dispenser to pump type for more sanitary use.
- Re-do the caulking around the bar hand sink.
- Turn off the ice machine, empty all ice out, remove all stains & mould, sanitize all surface and turn it back on.
Date to be corrected by: immediately; re-do the caulking until April 15, 2022.

Low temp dishwasher measured at 100ppm chlorine with test strips.
Glass dishwasher measured at 12.5ppm iodine with test strips.
QUAT sanitizer in spray bottles measured at 200ppm with test strips.
All coolers at 4C or below. *** Walk-in drink cooler not working - measured at 17C with infrared temperature gun. Manager stated work order has been put and technician is coming in today. No perishables observed in the cooler. Ensure the cooler can maintain the contents at 4C or below. ***
All freezer units ranged from -24C to -13C.
Premises has monthly pest control company visits. Manager to email the undersigned with the most recent pest report.
