Observation: - Live mouse observed on glueboard under bar beverage cooler.
- Rodent droppings noted in the following areas:
- Under shelving in dry storage room.
- Under beverage storage shelving in back hallway.
- Behind dough mixer and along window still behind prep cooler.
- Behind cook line equipment.
Corrective Action(s): - Remove mouse from premises.
- Remove droppings and thoroughly clean the above noted areas.
- Contact your pest control company for service. Follow all recommendations provided by your pest control company.
- Store all items at least 6" (15 cm) off the ground to facilitate cleaning and pest control monitoring.
- Inspect facility. Seal any holes and/ or gaps to prevent further pest entry.
- Maintain a higher level of sanitation throughout the premises to prevent potential pest attraction.
- Continue to monitor pest activity. Take corrective actions if new and/ or increased pest activity is noted.

Note: Operator contacted landlord to schedule pest control service at time of inspection. Next service is scheduled for November 28, 2017. Ensure that all areas of the premises are accessible for pest control service.
