Observation: Numerous (30+ live cockroaches observed on prep cooler shelf pole, under the whiteboard, in the dry storage shelf top shelf, and behind the 2 door undercounter cooler near the back of house sink
Pest control reports reviewed. Pest control is servicing the facility every 2 weeks however is noting that there is no activity in the traps during all past inspection reports.
Corrective Action(s): Vacuumed at the time of inspection. operator has started cleaning facility with staff.
Have the facility serviced by a different company or technician as it is clear that the current company or technician is incapable of doing a proper service of the facility and identifying pests.
Facility must be fully serviced by pest control service before July 4. Provide area inspector with pest control report

Observation: -walls around pipes going into walls noted to be gapped and have cockroach debris.
-cockroach debris noted around compressor of back of house undercounter cooler
-gaps noted with cockroach debris in the dry storage shelves
Corrective Action(s): Fill the gaps and clean areas thoroughly

Observation: 2 outstanding issues:
-grease trap requires pumping/servicing
-pooling water around the dishwasher
Corrective Action(s): have the grease trap pumped (operator indicated that this is to be done tonight) and repair the floor in the back dishwashing area to ensure there is no more pooling water.

-cleaning has been improved in the back of house however there is still further grease and debris build up noted around hard to reach areas such as behind shelves, under counters, and between cooler units and other equipment. Split kitchen up into sections and thoroughly clean the facility. due July 04.
