Corrective Action(s): Ensure all damaged equipment are repaired to original form or replaced to prevent possible physical hazard. Operator replaced strainer during inspection.

Observation: 1.Large hole cut into wall in back small storage room (left wall). Large cut wall piece available and appears to be awaiting mounting.
2. Back storage room ceiling tile has been moved and exposing open ceiling
Corrective Action(s): Ensure all surfaces in facility are smooth, durable, easy to clean and prevent pest entry.
1. Mount/ install/repair wall to close up wall.
2. Close up ceiling, re-locate ceiling tile.
