Corrective Action(s): Required operator to cover all food items in the walk-in cooler to prevent physical contamination of foods. Fan cover must also be cleaned regularly.

Observation: Screendoor in the back is present but needs repair. All screendoors must be installed so that it can prevent all pests from entering the facility.
Corrective Action(s): Required operator to repair the screendoor so that it can function as a barrier for pests. Until then, all doors must be closed.
