Observation (CORRECTED DURING INSPECTION): Sliced onions and green onions mix was stored in the toppings cooler at 9.7C, top layer. Sliced cucumbers were also stored on the top layer of the cooler.
Corrective Action(s): Ensure not to stack more than one layer of food in the top shelf of toppings cooler. Relocated cucumbers and onions/green onion mix to undercounter cooler.

Observation: 1) Several containers of fish sauce mixture (condiment) were stored on the kitchen floor. 2) An insert of raw chicken was being thawed and stored on the top shelf of the standup cooler by cooking station. Jugs of ice tea were stored underneath the raw chicken.
Corrective Action(s): 1) Keep food stored 6 inches off the floor. 2) Always store raw meats on the bottom shelf of coolers to prevent food underneath from being contaminated.

Observation: Several bags of frozen tendons are stored in grocery bags directly.
