Observation: 1. Walk in freezer has been converted to a walk in cooler with out approval
2. Multiple pieces of refrigeration have been moved around and are in new locations and large rotisserie was added with out approval
3. Multiple holes in the walls noted, which need to be filled (i.e. under dishwasher)
4. Multiple tiles are missing or broken on coving through out premises
Corrective Action(s): 1. Submit an up to date drawing of proposed floor plan for kitchen (including location of all equipment, coolers, and freezers) for review.
2. Repair all holes in wall and all missing/broken tiles
3. Repair missing or broken tiles on coving through out premises.

- Equipment and utensils have been properly cleaned and are being stored in a sanitary manner.
- Light in freezer, and electrical cord have been repaired
- Cutting boards have been replaced
- Sink and dirty side have been resealed
- Pest control has been contacted regarding fruit flies, will be onsite next week.
