Observation (CORRECTED DURING INSPECTION): 9 containers of cold potentially hazardous food, 1 bag of lettuce and inserts (sliced cheeses at 13.2C, 12.6C, cream cheese at 10.9C, sliced cheese at 11.7C, roast beef at 9.1C, herb and garlic cream cheese at 10.0C, sliced tomatoes at 9.6C, 10.0C, diced tomatoes and cucumber at 9.0C, shredded lettuce at 10.9C) were stored over 4.0C.
Sandwich prep cooler was malfunctioning at the time of inspection. All cold potentially hazardous foods held in this unit was dicarded during the inspection. Last time temperature was taken from this unit was more than 4 hours ago (at around 8:30 AM) and all food items were placed into this unit from this morning or previous night.
**Issue corrected during the inspection**
Corrective Action(s): As per Manager, unit will be replaced sometimes this week.
Due to the high turnover rate and volume of this store, small amount of cold food items may be stored within this unit with time stamp and discarded after 4 hours.
For any items that are not turn over as fast, they must be returned to another functioning refrigeration unit after 2 hours.

Observation: Fruit flies still noted in the baking area and mop sink area.
No improvement noted since the last inspection.

Corrective Action(s): Continue to work with your pest control company (Abell) to address the current pest issue. Pest company comes in once a month. May need to increase the frequency of inspections by your pest control company to address the issue if no improvements are noted during the next follow-up inspection.

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: **Note: This is a repeated violation**
1. Food soil still noted in the cabinet and below the bread holding rack in the baking area. Foods and doughnuts noted on the floor in the walk-in freezer. No improvment since the last routine inspection.
2. Buildup noted on the women's washroom mirror and soap dispenser. As per Manager from previous inspection, cleaning of the washrooms are conducted every half hour however when the sanitation personnel was asked when cleaning was done, it was done this morning at 8:30 AM (more than 3 hours from now).

Corrective Action(s): 1. Clean and sanitize the above noted areas of food soil and maintain in a sanitary condition to avoid attracting pests in the facility.
2. Clean and sanitize the washrooms, especially the high touch point areas, at the prescribed frequency as per your sanitation plan.

1. Sandwich prep cooler was noted at 8.5C and 7.7C during the inspection.
Same issue noted during the last routine inspection - The door of the prep cooler was not closed properly when food was loaded into the unit, causing a gap in the door that might have allowed hot air to enter.

2. No water available in the handwash station in the patron's women washroom.
As per supervisor, handwash sink has been not working for about a week. Issue called in. Waiting for a part to fix the handwash sink.

Corrective Action(s): 1. This to be a constant issue with this prep cooler. The door of the prep cooler must be properly fixed to prevent hot air from entering the unit. It must be able to maintain at or less than 4C at all time. If door cannot be fixed, consider replacing the unit.
2. Close off the women washroom and only let patrons used the men washroom until the sink in the women washroom has been fixed.

All hot hold units were noted > 60C.
All sanitizer containers were noted with 200 PPM QUAT sanitizer.

A further follow-up inspection will be conducted within 1 month from now. If no improvements are noted during the second follow-up inspection, progressive enforcement actions may occur.
