Corrective Action(s): Continue the contract with the pest control service. The technician visits once a month.

Observation: 1. The corner behind the grease trap foam was chewed off.
2. The mesh screen under the dishwashing sink was ripped.

2. Reinstall the mesh screen to block the entry point.
