Observation: Some rodent droppings observed in the following areas:
- Under ice machine
- Floor in hot water tank closet
- Bar floor
- Corner under shelf in far corner x2
Pest Technician report indicates ongoing infestation. New hole found and sealed.
Significant Fruit fly infestation in bar area

Corrective Action(s): Spray rodent droppings with 200ppm chlorine solution or equivalent. Remove droppings from area. Wet affected area with 200ppm chlorine or equivalent and soak area.
Follow recommendations by Pest Technician.
Continue to monitor and trap
Continue to inspect facility for new holes and seal immediately as currently doing
REMOVE BAIT/POISON from kitchen area - only traps permitted in kitchen or food storage areas
Consult with Pest Technician regarding fruit fly infestation. Remove/bag beverage containers immediately and do not let these items accumulate without bagging. Hang mop after use and do not allow item to sit moist

Observation: Beverage containers in front bar area are source of food for fruit flies. These items are piled up and unrinsed.

Corrective Action(s): Staff should bag up these items and ensure the bag is closed to prevent food source and breeding.
Mop should be hung to air out and dry out. Mops are an attractant for fruit flies

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: Cleaning required in the following places:
- Areas of rodent droppings (bar floor, floor under ice machine, under shelves in hard to reach corner X2, in hot water tank closet)
- Meat slicer
- Lower shelf under meat slicer has food debris
- Lower shelf under pizza prep where pizza pans are stored has food debris
- Containers storing slicing equipment and pizza pans have food debris that has fallen inside and requires cleaning.
- Walk-in cooler walls, ceiling, fan, floors, shelves
- Beverage dispensing nozzle in bar area has crusted black film inside - soak prior to use.
- Closet under the stairs requires cleaning and organizing to monitor for pests. It is currently impossible to determine if this is a source of the pest infestation

Corrective Action(s): Ensure thorough cleaning of facility. Food debris has accumulated in some areas and has the potential to attract pests (as does clutter)

Observation: Paper towel dispensing unit in bar area is a domestic type and unsuitable for commercial use. To use item staff must touch the top therefore recontaminating clean hands

- Sanitizer solution from dispensing unit at concentrations of greater than or equal to 200ppm QUAT
- Test strips available for use for both QUAT and Chlorine
- Two functional sink plugs available for use observed
- Ice machine scoop in good sanitary condition
- Proper cooling methods observed for previously cooked food items – use of large tubs for increased surface area
