Observation: Receiving back door entrance and seating area door both have gaps between door and floor that allow for entrance of pests.
Back corridor door for grease trap has a gap above weather strip that allows for entrance of pests.
Majority of snap traps throughout premise have been triggered and are currently disarmed.
Corrective Action(s): Replace back entrance door sweeps.
Install door sweeps to seating area entrance.
Install flashing to close gap between door and door jam leading to back grease trap.
Rearm all snap traps.

Observation: Upper floor storage areas - debris, food residue and mouse droppings observed on floor along walls / corners under shelving of dry storage room and under pop syrup shelving.
Floor / corners along wall behind equipment between front handsink and point of sale - food debris build-up.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.
Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine) after cleaning.
