Observation (CORRECTED DURING INSPECTION): At the time of inspection bleach sanitizer solution not prepared and available for sanitizing food contact surfaces.
Corrective Action(s): This is a recurring issue. Ensure during operation a 100ppm bleach sanitizer solution is prepared and available for use. Preparing and using the solution at the end of the day is not as per your sanitation plan or following food safe principles.

Observation: Cardboard used to line the bottom shelf in the lower section of the pizza preparation cooler. Cardboard also used to line the multi level shelving until by the walk in cooler.
Corrective Action(s): Do not use cardboard to line shelves. Cardboard can absorb moisture and is not easily cleanable. Remove the cardboard immediately.

Observation:
- Damaged strainer basket with duct tape used to store food.
- Chest freezer with damaged lid replaced with a new upright freezer. Chest freezer with damaged lid now used as a table.
Corrective Action(s): Ensure all food equipment is in maintained in good condition. Discarded the damaged strainer basket held together with duct tape. Remove from the premise the chest freezer with the damaged lid. Correct immediately.
